THE FORM

Indiana law requires a sales disclosure form (SDF) be reviewed for completeness by the county assessor​, for any property in the entire county, before being forwarded to the county auditor and must accompany any conveyance document. 

The following are recommendations and/or noteworthy reminders of items which many experience issues which may result in the form being rejected:

  • Legal Description (Section A)

    • Entire section needs filled out, unless mailing address is same as property address - that section can remain blank then.

    • For Number 7 use the building description, commonly referred to as the short legal. The full complete legal often times will not fit OR state "see attached legal" and add it to the back of the SDF.

    • If multiple parcels fill in page 5.

      • Each parcel must have its own legal description.​

  • Conditions​ (Section B)

    • All questions need answered 'yes' or 'no'.

    • If any of numbers 4-6 are checked 'yes' please note there is additional information needed.

  • Transaction Details (Section C)​

    • All questions need answered with the exception of number 8.

    • Conveyance date must match the date on the deed.

    • Number of parcels must match the number of parcels provided

    • For question number 10 please mark any that apply. It will instruct you as to which section or sections need to be filled out on page 2.​​

  • Residential or Agricultural Property Sales Data (Section D & E)​

    • This section is only to be filled out if the property is residential or agricultural.

    • All questions need to be answered.

      • The exception is if you answer no to question 6 then numbers 7 & 8 do not need to be answered.

    • If you wish for the information to remain confidential, you may check the box at the top of the section.​

  • Seller & Buyer (Sections F & G)​

    • This section is only to be filled out if the property is Commercial or Industrial

    • The italicized questions are not required, but are of great value to studies conducted.

      • All other questions need to be answered.​

    • If you wish for the information to remain confidential, you may check the box at the top of the section.​

  • Preparer (Section H)​

    • Must be filled out in its entirety​.

  • Seller & Buyer (Sections I & J)​

    • The names listed must match the names listed on the deed​.

    • Phone numbers are always necessary.

    • If the seller/buyer is a company, signatures need a title of the person written beside it.

    • If a Power of Attorney is signing for a buyer or seller, please provide either a recorded document number for the POA or attach a copy of the POA to the sales disclosure form.
